Worked example: SCR and equivalent records
English and Welsh GP terms require defined summary information to be uploaded or made available for automated retrieval when records change. England's applicable NHS service contract separately requires viewing capability for urgent, emergency and unplanned care. Scotland requires care-related ECS/KIS requests and controls while allowing appropriate means; the reviewed NI sources establish services rather than a universal summary-upload law. 183, 184, 181, 195, 211, 220, 226
The organisational implication is to assess three responsibilities separately: the GP source, the receiving service's capability, and the individual user's authority. A connected system does not settle all three.
Follow the information from GP to care team
An urgent-care clinician needs current medication and allergy information held by a GP. This example concerns the person's care. The applicable GP arrangement and receiving-provider contract are conditions to establish, not assumptions that every provider has the same duties.
| Step | Responsible organisation and information | Digital action and timing |
|---|---|---|
| 1. Maintain the defined summary | The relevant GP contractor holds medication, allergy and adverse-reaction information; additional summary data has its own conditions. | English terms require automated SCR upload; Welsh terms require automated summary retrieval through approved systems when record information changes. |
| 2. Provide the care-team route | The receiving care service needs the relevant summary, rather than an automatic entitlement to the whole GP record. | Applicable English service terms require key GP-information viewing capability. Scottish Boards must request ECS and existing KIS when necessary for care; electronic means are one permitted method. |
| 3. Authorise the particular user | The clinician must have the appropriate care role and justified access to the information. | Apply confidentiality, data protection and service permissions. Technical enablement does not establish every user's access entitlement. |
The source and receiving-provider duties arise through different instruments. The national detail below identifies them; the requirements catalogue records the exact actors, methods, triggers and limits.
Four-nation exchange map
“SCR or equivalent” is a comparison of function. Each service retains its own dataset, scope and legal route.
| Nation and service | Identified action and timing | Legal force and boundary |
|---|---|---|
| England: Summary Care Record (SCR) | General Medical Services (GMS) regulation 68 and Personal Medical Services (PMS) regulation 61 prescribe automated upload of the defined summary when the patient's medical-record information changes. | Prescribed GP terms. Medication, allergy and adverse-reaction data form the summary; additional data has agreement/consent conditions and exclusions. ENG-001. 183, 184 |
| England: GP Connect and receiving-provider access | Defined GP contractors must enable the HTML, Structured and Update Record functions and take reasonable steps to keep them operational. Applicable NHS service terms separately require urgent/emergency/unplanned-care staff to be able to view key GP information through SCR or an integrated record supplemented by SCR. | Different GP and receiving-provider duties, with their actual contractor/service scope. ENG-002, ENG-010. 183, 184, 181 |
| Wales: Welsh GP Record and NHS Wales App | GMS Schedule 3 paragraph 79 requires approved systems to enable automated retrieval of the defined summary when medical-record information changes. | Prescribed contract term. Approved systems, summary scope, consent/agreement qualifications and clinical-use boundaries apply. WAL-001. 195 |
| Scotland: Emergency Care Summary (ECS) and Key Information Summary (KIS) | The 2023 Directions require necessary Board requests for ECS and existing KIS; the Board chooses appropriate means, including electronic means. Access/security controls also apply. | Binding request/control framework; not an exclusively electronic upload requirement or a duty to create a KIS for every patient. SCT-D05, SCT-D08. 211, 212 |
| Northern Ireland: NIECR, encompass and professional access | Published services provide summary/wider-record capabilities. Approved professionals can use EpicCare Link for specified viewing and limited actions. | Operational access is established. These descriptions do not prove a universal GP-summary upload law equivalent to the English or Welsh terms. NI-D08. 220, 226 |
Summary content and other record routes
A blood result or discharge letter may be available in a wider record without forming part of the mandated GP summary. Trace its own exchange through the blood-results or discharge example, rather than treating the summary duty as a whole-record obligation.
A patient portal also serves a different role from a professional summary viewer. NI My Care is optional and displays selected encompass information with stated exclusions and release conditions. It is not a complete GP or social-care record and does not replace information-access rights. 225
Access, representatives and independent providers
Care-team access differs from a parent's, attorney's or other representative's proxy access. Establish the actual authority and permitted information separately. The capacity and representation map explains that distinction alongside confidentiality and professional direct-care guidance. 006, 174
GP terms apply through the defined national contract. An independent practice delivering covered publicly arranged care can be within that scope; wholly self-pay work does not acquire the same contract through ownership or registration. Scottish further-access arrangements also need the relevant agreement and care purpose. 183, 195, 211
Assurance conclusion
For the English and Welsh GP rows, the prescribed digital action and actor are identified. The Scottish framework requires care requests/access controls but permits more than one means. The cited NI material establishes services, with any stronger local or contractual duty still requiring its own evidence. Record that distinction when approving a summary-record connection.
