Research log
3 November 2026 — protection concerns throughout care
Clarified the homepage and related reader, concept, discovery and maintenance text: protection concerns can arise at any point in care. Hospital discharge remains one worked setting for the existing child/adult cases. General workflow descriptions now make any care handover conditional. The specific scenarios, legal findings, source identities, evidence states and requirement counts remain unchanged. Earlier research records retain their original description of the discharge examples. Verification and republication are recorded separately in the release evidence.
3 November 2026 — deployment fixture isolation
The guarded production run stopped before upload when its publication-test fixtures inherited the real Cloudflare account setting. Their simulated account intentionally differs, so the unchanged target guard refused them. The helper now has an explicit environment dependency with the host environment retained as the production default. Test fixtures use an isolated environment; a separate negative test verifies that a conflicting account is still refused before commands run. Publication tests are checked both with and without the real account selection before renewed full verification and release.
3 November 2026 — mobile architecture diagram correction
The first GitHub Linux browser run reported 71 pixels of horizontal overflow for the architecture diagram at a 390-pixel viewport, above its 60-pixel allowance. Local browser verification had passed. Publication was stopped before upload. The diagram now groups the three derived views in one node, preserving the source-to-knowledge-to-views relationship with less horizontal breadth. Full local verification and a new exact-commit GitHub check are required for the corrected release.
3 November 2026 — verified reader-journey proposal provenance
The editorial/protection implementation passed all nine verification gates and independent maintainer provenance, then was committed as 4e17d4e6e01d51e24137e1ea9b5514e98627ac02. The private GitHub source repository was created and its verified source pushed to main. A tracked schema-valid reader-journey proposal records the full originating commit, JJ change, immutable Standard pin and verification summary. Its scrubbed payload was prepared and inspected locally. The governed issue form remains unsubmitted; the separately solicited Standard PR does not imply acceptance, release or adoption. Source-label typography was corrected without changing source identities, URLs or legal findings.
3 November 2026 — answer-first review, protection extension and publication preparation
Reviewed all 38 Human Wiki pages and 15 concepts at the pre-review baseline, then compared six local wiki/Standard implementations. Applied an answer → comparison → example → national/legal detail → exact record → evidence reading journey. Boards and senior decision-makers lead; specialist depth remains linked. Source retention and independent editorial/source-fidelity reviews preserved legal force, recipient scope, timing and uncertainty. The project-owned authoring policy records this manual pattern; automated structural checks do not certify narrative quality.
The owner confirmed public-use permission for Parliament marks, private GitHub source/public website visibility, retained reuse rights, the name UK Digital Healthcare Legal Requirements, and the selected Cloudflare Pages origin. Publication uses a separately checked static build; the managed local preview retains its registered identity. The immutable Standard pin remains unchanged. The reusable editorial pattern is prepared for a solicited Standard PR, with no claim of merge, release or adoption.
Added child/adult protection detail, two four-nation discharge examples and a companion catalogue of 40 assessed exchanges and contexts. Relevant police and education sharing is included. Seventy-two new sources bring the register to 268; 23 additional grouped claims bring the evidence matrix to 108; the portable bundle now has 16 concepts. The 56 core catalogue records remain counted separately. Statutory reporting, permissions, digital contract/system requirements, regulator routes and guidance retain their distinct force.
Current findings include England's September 2026 child-sharing duty and separate CP-IS contract/standard, Welsh relevant-partner reports, Scottish public/private reporting distinctions and repealed named-person provisions, NI's August 2026 CJSM protocol and regional electronic recording, plus selected specialist notices. Protection disclosure and representative access remain separate decisions. Current protected forms, local adoption/deployment and future-law milestones retain explicit limits. No real patient information or external correspondence was used. Technical verification and publication outcome are recorded in separate generated release evidence.
3 November 2026 — national service-report comparison
The user authorised one national planning/activity-report example per nation and asked what the proposal meant. Reader prose now defines a return as a report. The new comparison covers English CDS outpatient reporting, Welsh GP appointment extraction, Scottish CGS assessment claims and NI KH03a bed/non-acute reporting, with information, sender, recipient, electronic action, timing, authority and private-provider scope.
Fresh primary checks and independent review preserve the legal distinctions. Welsh monthly review is not an upload deadline; Scottish claim terms apply when making a claim, with a three-month period rather than 90 days. The general NI Trust reporting duty is verified, while the specific KH03a digital legal linkage remains qualified. SMR01 is retained as an operational electronic collection and six-week target; no collection-specific binding digital authority was found in the bounded search. This does not prove none exists. Neither the NI quarter-specific submission date nor the SMR target is converted into a universal deadline.
Sixteen new source records bring the register to 196. The catalogue has 56 rows: 22 identified digital obligations, 14 qualified mandates/conditions, ten medium-neutral duties and ten operational/recommended routes. Three existing obligations were reused, not counted as new. Six additional bounded matrix claims bring the total to 85. VAL-014 is complete for this comparison; unresolved legal links are parked with evidence triggers. No messages were sent to external bodies and no recurring monitor was created. Technical verification is recorded separately by the generated receipt/report and end-of-turn JJ phase.
3 November 2026 — legally required digital sending and availability
The user clarified the main question through sequential questions and authorised a rewrite around what healthcare organisations must legally send digitally or make digitally available. Individual care is primary, but planning, research and public health are also covered. Healthcare/social-care organisations lead; supplier material supports their duties. The home question, navigation, four national chapters, comparison, legal frameworks, portable concepts and agent discovery now reflect that focus.
The new table assesses 54 rows: 22 identified digital obligations, 13 qualified standards/access conditions/national mandates, ten information duties without a digital-only method and nine operational routes/recommendations. Each row records information, sender, recipient, digital method, timing, authority, force and provider/territorial limits. Published national contract terms are distinguished from unseen local agreements and from statutory provisions. The corpus now contains 180 sources, 79 bounded matrix claims and 15 concepts.
The three worked examples cover SCR/equivalent records, blood tests/results and hospital discharge. The four-nation discharge map adds capacity, children's care and representative authority. Peer review checked source fidelity, the English SC11.5 GP and/or Referrer qualifier, the English/Welsh age-18 LPA donor rule, current Scottish CGS Directions, and NI's partial capacity-law implementation. Care authority, confidentiality and portal access remain separate questions.
Local discovery screened 3,193 canonical text files across 52 project folders, identifying 46 wiki roots and 26 prioritised leads. Selected source rows received deeper review; this was not a semantic read of every paragraph. Primary official sources were independently checked for published legal conclusions. A dated project-relative discovery trail and requirement dataset are retained under research/.
Current Welsh Control Standard v5 resolves the operative-version gap, while v4 remains historical. WHC2026/031 reporting has a 31 March 2027 implementation date. The pathology timetable remains qualified: an announced extension without a replacement date is not a new deadline; April 2027 pilot assurance is not nationwide conformance. Previously resolved Scottish/NI reform status was preserved, not reopened.
A graph query preceded research, with canonical fallback. The reader title is UK Digital Healthcare Requirements; the managed service identity, port, immutable Standard pin, Parliament palette, current icon and favicon remain unchanged. Live home/table/example checks confirm the new reading journey. Full technical verification and provenance are recorded separately by the end-of-turn JJ phase and generated receipt/report.
3 November 2026 — individual-care narrative and structure rewrite
The user said the wiki's intent had become unclear. Six sequential questions established the subject: laws governing digital patient-information sharing for an individual's care, including healthcare/social-care exchange, with publicly funded healthcare primary and private healthcare compared. All four UK nations and responsibility, legal-framework and national views are required. The user explicitly selected a narrative and structural rewrite.
The home question, introductory narrative, navigation, shared chapters, national accounts, care examples, portable concepts and agent/discovery metadata now reflect that scope. New responsibility and legal-framework pages connect the explanation. Earlier research and implementation proof remain in labelled context; the completed Scottish/NI checks were not reopened. General IT/device detail no longer leads the account.
Twenty-four new source identities strengthen the care narrative. Exact legislation, current national guidance and professional standards distinguish statutory duties, powers, confidentiality, data protection, public-care definitions and private-provider responsibilities. The corpus now has 116 sources, 47 bounded claims and 13 concepts. Scoped peer reviews checked national/shared source fidelity, including English provider definitions, Welsh information/cooperation provisions, Scottish integration and contracted-care principles, and NI interagency consent guidance.
A current graph query preceded the rewrite. Primary law and national official sources supplied the analysis; graph edges created no legal conclusions. The amended Articles 6 and 9 were retrieved as official XML; new principles, rights and controller-responsibility sources were read in current official text.
The repeated menu-colour request was checked in the live renderer. The question and active Home already match exactly: light #f2f0ff, dark #2b2937. No colour change was needed. The reader title changed to UK Care Information Sharing Law; service identity, Standard pin, logo and palette remain stable. Required technical verification is recorded separately in the generated receipt/report and end-of-turn JJ phase.
3 November 2026 — implementation status and reader presentation follow-up
The user authorised resolution of the Scottish and NI status checks, updates to the national pages/register/matrix, four browser design comments, and the source page's logo/favicon.
The Scottish review checked dated Part 1 and individual-section XML, the complete 60-row effects table, official instrument searches and programme material. Sections 2–7 are explicitly prospective; section 1 is in force from 7 January 2026. The NI review checked the Act and all three recorded effects, the current Assembly Bill and Health Committee trackers, official questions and instrument registers. The Act commenced on 12 April 2016, while its implementing approval machinery remains pending on the published official record. Publication coverage and editorial lag remain explicit limits. Both national updates passed an independent peer review.
The shared corpus now has 92 registered source identities and 28 bounded evidence claims. VAL-001/002 preserve resolved current positions and specific reopening events. No recurring monitor was created. A graph query preceded source research; truncated graph recall was followed by scoped canonical records and primary official sources.
The current local Standard, pinned rc.9 and latest registered wiki DSRF were compared for header treatment. The consumer adopts bounded header-control refinements without changing its frozen release/dependencies. Equal question padding and question-tinted active navigation implement the browser comments. Exact SVG/ICO assets were bundled from the rendered Parliament page; original bytes and hashes are retained. The mark's separate terms and independent/local-only boundary are recorded.
3 November 2026 — initial baseline
The user selected UK digital health/healthcare IT, interoperability and information exchange across all four nations. Publicly funded healthcare organisations are the primary audience; private providers and suppliers are compared. Authoritative external research was authorised. The user selected Parliament's purple colour reference.
Research ran in separate national streams with a second fidelity review. The corpus registers 76 sources and 27 bounded evidence claims. Principal findings distinguish organisational authority, UK GDPR, confidentiality, national standards, clinical/device safety and public accountability. Scottish commencement, NI implementation, forthcoming reforms and incomplete specialist layers remain explicit.
The England review corrected the location of the relevant IT-provider definition: section 250(7), while section 250A concerns the content of IT standards. Statutory XML confirmed the amended Article 6, including recognised legitimate interests and the public-authority task restriction. General GMC wording about the NI 2016 Act was retained with a later implementation caveat.
The release resolver verified Standard v0.1.0-rc.9 at 8800e139d73fd58ee1bd756d3e5497f62ed678e4, with successful exact-SHA Verify push/main run 40606700715, attempt 1. The frozen reference tools and locks were installed. A new project-owned service was reserved on port 8035. No remote publication is configured.
Graph discovery was unavailable at the empty-workspace start, so research began with official sources. The generated graph will index the maintained corpus; future work starts with graph queries and falls back to canonical pages when recall is insufficient.
Technical verification results are recorded separately in the generated receipt and paired conformance report. No source/build success alone is claimed as rendered or live-service proof.
